Scheduling Your Appointment
Before scheduling your appointment with East Alabama Neurosurgery & Spine, please review the following requirements. Having the necessary imaging and documentation ready will help us review your referral and avoid delays in scheduling.
What You’ll Need Before Scheduling
Recent MRI or CT
You must have an MRI or CT scan of the area of concern performed within the past year.
Previous Office Notes
We require your most recent office notes documenting the conservative treatments you have tried, such as physical therapy, medications, or injections.
What to Bring to Your Appointment
Recent MRI or CT
Please bring a copy of your MRI or CT imaging disc to your appointment so your provider can review the images. You can obtain a copy of your imaging disc from the facility where your MRI or CT scan was performed.
Photo ID & Insurance Card
Please bring a valid photo ID and your current insurance card.
